Application for short-term change of training and nursing method by Institute students Case Description This applies to Institute students whose parents would like to change their children’s training or nursing approach on a short-term basis due to family emergencies or other matters. Required Documents Submit relevant supporting documents (e.g. proof of medical conditions or treatment and proof of employment) How to Apply Fill out the Application Form, attach the required documents and apply via one of the following methods:
1. In person
2. By fax
3. By mail
4. Via the Internet by visiting Taipei e-services online (Chinese)
Processing time 10 days Office accepting applications and inquiry telephone number Social Work Section Assessment Procedure for Admission Required documents received→ Home visit by social worker→ Case review by a assessment team→ Case accepted or rejected.
